Your registration fee includes:

Fees for both the Primary Care and OB/GYN Conferences are the same.

ON-SITE REGISTRATION
Only on-site registration will be available after March 10,2008. We regret that we are unable to guarantee a course syllabus. An additional $50 fee will be charged for on-site registrations. Cash, check or credit cards(additional 5% fee) are accepted for on-site registration.

CONFERENCE CHECK-IN
For all Kaiser Permanente Clinicians: Please bring your official Kaiser Permanente ID badge.

For non-Permanente: Please bring a photo ID and a conference badge will be issued to you.

 

CANCELLATION POLICY

  • All cancellations must be submitted in writing either via fax or e-mail.
  • $50 cancellation fee if notice is received before February 11,2008. Refund process can take 3-4 weeks.
  • No refunds if notice is received after February 11,2008.
