*Residents in Training must provide proof of Residency from their Director. Please send the form to CMX Travel via fax or e-mail.  

Course Refund Policy

  • All cancellations, regardless of reason, are subject to a $100 cancellation fee
  • A $100 administrative fee will be refunded if written notification is received before the deadline date of August 5, 2017
  • We regret that we are unable to honor refunds after the deadline date of August 5, 2017.

You may e-mail or fax your cancelation notice to CMX Travel & Meetings. Canceled registrations are non-transferable.
