PLEASE NOTE THAT A 5 NIGHT MINIMUM IS REQUIRED OVER THE PROGRAM DATES OF JULY 27-02 AUGUST, 2008 AND CONFERENCE REGISTRATION IS REQUIRED.

RESERVATION PROCEDURE
We are pleased to offer this 8-day, 7 night conference at the Spectacular Grand Hyatt Kauai Resort and Spa.

For room reservations, please contact CMX Travel and identify yourself as a 2008 Update In Orthopaedics’ Conference attendee. Rooms are available to conference registrants, their families and guests and corporate sponsors only through CMX Travel and are limited in number.

CUT-OFF DATE
The "cut-off date" is June 27, 2008. Reservation requests received after the cut-off date will be accepted on a space and rate availability basis up to the Group block. If the group rate is not available after the cut-off date, the prevailing rack rates will apply for any reservations confirmed.

POLICIES/PROVISIONS

• Rates are valid between the dates of from July 24 to 04 August, 2008 inclusive-but subject to availability.
• Please note that Kaiser Permanente does not block rooms pre and post of the official conference dates.
• All rates are based on single or double occupancy and do not include applicable hotel tax currently at 11.416%,
• baggage handling fees of approx. $8.00 per person round trip plus sales tax
• Resort Services Fee of $15.00 per room, per day
• All rates are based on single or double occupancy, European Plan.
• For additional persons 19 years and older, a $35 charge per night is applied.
• Maximum four (4) adults or two (2) adults and two children per room.

• A two night deposit is required to confirm a reservation.
• Conference registration is required and rooms may be canceled if you are not a conference registrant, faculty member or corporate sponsor.
• A 1 (one) night cancel fee applies for any reservation canceled after initial deposit
• Seven (7) days up to Arrival Date: Cancellations and no-shows received within this time period will be assessed the full stay penalty, plus Hawaii State tax.
• Changes to your arrival/departure date are subject to a $50 per change fee.

• One-Hour Court Time Per Day At The Tennis Garden Reservations are required. Usage may be limited due to demand. Equipment rental is available at the Pro Shop.

CHECK IN/CHECK OUT TIMES
Guests may check into the hotel after 3:00 p.m. on the arrival date and check out no later than 12:00 Noon. Anyone arriving earlier than 3:00 p.m. will be checked in as soon as rooms become available. Anyone wishing special consideration for late check out should inquire at the front desk on the day of departure.

EARLY DEPARTURES
Should Group or any individual traveling with the group check out earlier than the confirmed dates, the Hotel will retain the two (2) nights’ deposit for the balance of the scheduled stay. In the event of an unforeseen personal emergency, the Hotel will review these on an individual basis.

UPON DEPARTURE
Each individual will be responsible for their incidentals and the balance of their room charges upon check out. Credit card imprints will be taken upon arrival for settlement of individual accounts.
