Reservation Procedure
For room reservations, please contact CMX Travel and identify yourself as a 2012 Update in Orthopaedics' Conference attendee. Rooms are available to conference registrants, their families and guests and corporate sponsors only and are limited in number.

Cut-Off Date
The "cut-off date" is June 15, 2012. Reservation requests received after the cut-off date will be accepted on a space and rate availability basis. If the group rate is not available after the cut-off date, the prevailing rack rates will apply for any reservations confirmed.

Policies/Provisions

• Rates are valid between the dates of from July 26 to 05 August, 2012 inclusive-but subject to availability.
• Please note that the conference does not block room's pre and post of the official conference dates so availability is limited.
• All rates are based on single or double occupancy and do not include applicable hotel tax currently at 13.42%,
• baggage handling fees of approx. $9.00 per person round trip plus sales tax (subject to change)
• Resort Services Fee of $15.00 per room, per day
• Valet Parking is $10; self parking included in Resort Fee
• All rates are based on single or double occupancy, European Plan.
• For additional persons 19 years and older, a $50 charge per night is applied.
• Maximum four (4) adults or two (2) adults and two children per room.

Deposit/Cancellation Policy

• A two night deposit is required to confirm a reservation.
• Conference registration is required and rooms may be canceled if you are not a conference registrant, faculty member or corporate sponsor.
• A 1 (one) night cancel fee applies for any reservation canceled after initial deposit/reservation
• From 45-7days prior to arrival; 2 night cancel fee applies
• Seven (7) days up to Arrival Date: Cancellations and no-shows received within this time period will be assessed the full stay penalty, plus Hawaii State tax.
• Changes to your arrival/departure date or name changes are subject to a $50 per change fee.

Resort Service Program $15.00 Per Day
The resort fee program covers: Access for two (2) to the fitness center including fitness classes, internet access, self parking, two (2) bottled waters in-room daily, in-room coffee and tea, 10% discount on select merchandise at Anara Spa, the Logo Shop at the Tennis Garden, Poipu Bay Golf Pro Shop and Captain Andy's Lagoon Side Hut, One (1) hour tennis court time each day, 10% discount on business center internet access, local and 1-800 phone calls, incoming faxes, local newspaper, luggage tag made with key upon checkout.

Check In/Check Out Times
Guests may check into the hotel after 3:00 p.m. on the arrival date and check out no later than 12:00 Noon. Anyone arriving earlier than 3:00 p.m. will be checked in as soon as rooms become available. Anyone wishing special consideration for late check out should inquire at the front desk on the day of departure.

Early Departures
Should Group or any individual traveling with the group check out earlier than the confirmed dates, the Hotel will retain the two (2) nights' deposit for the balance of the scheduled stay. In the event of an unforeseen personal emergency, the Hotel will review these on an individual basis.

Upon Departure
Each individual will be responsible for their incidentals and the balance of their room charges upon check out. Credit card imprints will be taken upon arrival for settlement of individual accounts.
