RESERVATION PROCEDURE
We are pleased to offer this 5-day, conference at the Spectacular Grand Wailea Hotel Resort and Spa.

For room reservations, please contact CMX Travel and identify yourself as a 2009 Update in Orthopaedics’ Conference attendee. Rooms are available to conference registrants, their families and guests and corporate sponsors only through CMX Travel and are limited in number. Toll free 1-877-843-8500.

CUT-OFF DATE
The "cut-off date" is June 25, 2009. Reservation requests received after the cut-off date will be accepted on a space and rate availability basis. If the group rate is not available after the cut-off date, the prevailing rack rates will apply for any reservations confirmed.

DEPOSIT AND CANCELLATION POLICIES
Two night deposit at time of reservation.

Cancellation: One (1) night cancellation fee after initial reservation, 2 night cancel fee within 30 days prior to arrival.

Ancillary charges subject to change.

Policies

RESORT CHARGE
A Daily Resort Charge of $20.00 (plus 4.166% tax) per room, per night includes:

• A floral lei greeting upon arrival.
• A welcome Mai Tai punch (non-alcoholic) at the designated check-in area.
• Unlimited local, toll-free, and credit card calls (no access fee).
• Internet access in all guestrooms. Built-in Wireless Internet access in all suites.
• Coffee or tea in guestroom or suite.
• Nightly turndown service. Unlimited use of Spa Grande's cardiovascular and strength training facilities.
• Participation in a variety of Yoga classes. Admission to twice-daily scuba clinics.
• Participation in scheduled Garden and Art tours featuring the works of Botero, Leger, Picasso and Warhol.
• Use of self parking facilities. Shuttle service to scheduled stops throughout the Wailea area.

TAXES
All rooms are subject to applicable taxes. The current Hawaii State Hotel Tax is 11.416%. (4.166% Hawaii State Sales tax and 7.25% Transient Accommodations tax). Hawaii State & Transient Accommodations tax are subject to change.

OVERNIGHT VALET PARKING
$15.00 per car, per night (plus 4.166% tax). Charges will automatically be posted to the guest's incidental account. Self-parking is included in the Resort Fee.

CHECK IN/CHECK OUT/REGISTRATION
Check in is after 4 p.m. Check out time is by noon. Must be 21 years of age or older to register. Proper identification will be required at time of registration.

ADDITIONAL PERSON (more than 2)
Guest Room & Suites: $50.00 charge per night for additional adult. No charge for children 17 & under when using existing bed space. Maximum occupancy per room is four (4) persons.
Napua Tower & Suites: $100.00 charge per night for additional adult. No charge for children 17 & under when using existing bed space. Maximum occupancy per room is four (4) persons.

ROLLAWAY BED
$30.00 charge per night for a rollaway bed.

GUARANTEED CONNECTING ROOM REQUEST
To guarantee connecting rooms, please contact Grand Wailea Resort reservations for availability. A charge of $25.00 per room ($50.00 for the two rooms) per night is applicable.

Connector Rooms: Guestroom interior door between (connecting) two guestrooms.
Adjoining Rooms: Guestroom nearby (NO connector door in-between guestrooms).
